The insurers SegurCaixa Adeslas, Asisa and DKV ultimately did not agree to participate in Muface’s next concert. DKV is also leaving Mugeju, while the other two companies are inviting tenders for the provision of healthcare to Isfas and Mugeju.
DKV He explained that he had made this decision “constrained by the insufficiency of the economic proposal, which would lead to a deterioration in patient care and a deterioration in the remuneration of health professionals”.
For his part, Adèslas indicated that he would attend the Isfas and Mugeju concerts because “in both cases, the parameters linked to the evolution of health demand allow a balance between the financing offer carried out by the Administration and the expected health expenditure”.
The Government gave the green light to the mutual insurance agreement last October. The new conditions envisage a 17.12% increase in premiums and a duration of two years. Thus, the total amount of the contract amounts to 1,337,059,970 euros in 2025 and 1,344,553,098 euros in 2026. In total, 303.9 million euros more compared to the last year of validity of the previous agreement.
These figures are not enough for DKV to continue to offer its services within the mutual insurance company. “Losses in the event of renewal would amount to 77 million euros over the next two years, almost as much as during the three years of the previous agreement,” explained the insurer.
The company assures that the model is underfundedsince the medical expenses during the last concert were more than 10% higher than the bonus he received. Furthermore, “over the last ten years, health expenditure per capita of autonomous communities and collaborating mutual societies has increased by 60.6%, while the weighted Muface premium has only increased by 31.6%, i.e. almost half the increase,” he continued.
Thus, for the first time since the creation of mutuals, DKV will not be part of the insurers which provide services to mutualists, after having accumulated losses, before taxes, of approximately 70 million euros.
The new concert
The 2025-2026 concert offers a bonus of 17.12%, distributed in a manner 16.5% in 2025 and 0.62% in 2026. With this increase, the average premium per mutualist per year will amount to 1,032.12 euros. to 1,208.81 in 2026which represents an increase in the premium per insured of 176.69 euros.
As the Ministry of Digital Transformation and Civil Service explains, they took two concepts into account when calculating the bonus. On the one hand, it covers new health services included in the portfolio of the national health system (such as oral health or innovative drugs and medicines). advanced therapies) and the increase in the amount of medical fees.
And, on the other hand, the tariff deficit (the difference between the premiums paid and the health costs borne by the entities), in order to calculate a premium increase to compensate for losses and meet new expenses of the health system.
According to the latest figures provided by DKV, the three insurers combine losses worth 470 million euros given the increase in costs in recent years.
Add to this the fact that, while public investment per capita has increased by 53% over the last ten years, that allocated to mutual societies has only increased by 32%, according to data from the IDIS Foundation. In addition, The average mutual premium remains lower than public spending.
